  • Blind or disabled individuals who require home care
  • Residents of state-run adult homes
  • People who participate in foster programs
  • Individuals screened for Presumptive Eligibility
  • Individuals eligible for COBRA, the Medicare Savings Program, the AIDS Health Insurance Program, or the Medicaid Buy-in Program for Working People with Disabilities. 

These individuals must apply for CDPAP through the Local Department of Social Services or a Facilitated Enroller for the Aged, Blin, and Disabled.
