What is the NHTD Waiver Program?

The Nursing Home Transition and Diversion (NHTD) waiver program uses Medicaid funding to provide support and services to individuals with Alzheimer’s or dementia.

These services provide an alternative to institutionalization and are meant to assist the individual in living their day-to-day lives while being an integral part of society and their community. The individual is the primary decision-maker and works in cooperation with a team of providers to develop unique strengths, health, welfare, goals, needs, and choices.

The NHTD Waiver Program leads to personal empowerment, independence, community inclusion, self-reliance, and meaningful activities.

What is HCSS?

Home and Community Support Services (HCSS) is an individually designed service run through the NHTD waiver program which provides the necessary level of support, oversight and/or supervision to assure the health and welfare of participants living in the community. Oversight and/or supervision may be needed for safety monitoring to prevent an individual from harmful activities. Oversight and/or supervision can be accomplished through cueing, prompting, direction and instruction.

How does HCSS differ from HHA services?

HHAs can administer personal care duties as well as limited medical procedures.  HCSS workers provide oversight/and or supervision to a participant in addition to PCA or HHA services (if needed). If the applicant/participant does not require oversight and/or supervision, HCSS would not be an appropriate service. Personal care assistance can be provided to the participant in addition to oversight and/or supervision, but the key element in HCSS is the need for oversight and/or supervision.

Our experts will review your individual needs and help you asses all of your options. We will act as the link between you and the insurance company or managed care provider, ensuring a smooth transition from enrollment to authorization of services. Our goal is to provide the highest level of home healthcare with the least financial burden to the client and family. We facilitate all insurance claims and process all paperwork to simplify the process.

Quality Healthcare participates with all major health insurance plans as well as Private Insurance, Life Insurance-Long Term Care, Medicare, Medicaid, and Managed Long Term Care (MLTC) to help cover the cost of your home care. Quality Healthcare also accepts all major credit cards and private payments.
