that is dedicated to promoting highly personalized care to clients in
their homes with services that are designed to promote wellness, safety,
and independence. Services include:
Nursing
Services

Registered
nurses (RNs) provide a broad range of nursing services based on a thorough
assessment of the clients health care needs and development of
an individualized care plan. Examples of services provided include skin
care, medication management, diet, nutrition management, and care focused
on a particular diagnosis such as diabetes, dementia and others. Physician
orders and efficient care coordination are integral aspects of nursing
services.
Rehabilitative
Services

- Physical
Therapists (PTs) . . . provide therapeutic exercise and treatments
to restore mobility, reduce pain, and maximize function. PTs trained
in providing services in the home adapt the environment to the clients
needs and also instruct in the use of equipment. Client independence
is at the heart of this service.
- Occupational
Therapists (OTs) . . . perform therapeutic exercises and
instruct in activities such as meal preparation, toileting, and grooming.
OTs also assist with energy and endurance problems, and address equipment
needs such as grab bars. Self reliance is always a goal.
- Speech
Therapists (SLPs) . . . provide treatments to improve speech,
swallowing, communication, and problem-solving skills. Often applied
with other therapies, speech therapy significantly contributes to
stabilization and recovery.
Supportive
Services

- Home
Health Aides (HHAs)
. . . provide services such as bathing, skin care, hair care and a
variety of services directed toward comfort and safety. Our HHA services
are always nurse supervised. Meal preparation, grocery shopping, or
light housekeeping are generally provided in conjunction with personal
care services. Short or long hour arrangements are tailored to the
client's needs.
- Personal
Care Aides (PCAs) . . . offer companion, homemaker, and personal
care services. PCPs assist with bathing and toileting, socialization,
run errands, write letters, and help with day to day activities. Dependability
can be expected. These services are often the difference between success
and failure of seniors remaining in their own homes.
Medical
Social Services

- Medical
Social Workers (MSWs) . . . medical social workers assist
with community resource information for needs such as housing, finances,
transportation, caregiver respite, and the need for additional care.
MSWs also provide assessment and counseling directed toward improvement
of coping skills, stress reduction, and depression/anxiety.
Case
Management Services

- Case
Managers . . . employ a multidisciplinary approach to work
with clients and their families in developing a plan to meet their
health care and daily living needs. Complex situations may require
coordination among numerous care providers, legal, and financial representatives.
An outstanding written and verbal communication loop with families,
physicians, and others underscores this service. This service offers
peace of mind to families who may not be familiar with available resources
and services or who reside in distant locations.
Referral
Process and Costs

- In addition
to private pay, Quality Healthcare accepts payment from most insurance
companies, including many long term plans. We will provide a free
insurance evaluation and will assist you in getting the most care
with the least financial burden possible.
- Our
office is open from Monday through Friday from 9:00 AM to 5:00 PM
E.S.T., but we are available 24 hours a day, 7 days a week to handle
any emergency or request for care.
